JOB DESCRIPTION SUMMARYResponsible for managing all aspects of the patient intake process including managing the members of the intake team, establishing and maintaining positive relationships with customers and referral sources, responding to customer requests and concerns, and managing the insurance verification and authorization processes.ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES1.Directs all daily patient referral and intake operations including providing direct oversight of the establishment and implementation of intake policies.2.Checks insurance eligibility of potential patients.3.Calls referral partners with clinical questions and clarifies clinical orders.4.Follow up with patients primary care physician and patient.5.Introduces Meridian Home Health to patient and insures they are aware that they will be called by a clinician to begin care.6.Ensures compliance with all state, federal, and Joint Commission referral/intake regulatory requirements.7.Directs the implementation of improved work methods and procedures to ensure patients are admitted in accordance with policy.8.Establishes and maintains positive working relationships with current and potential referral sources.9.Ensures seamless transition of patients to home care.10.Ensures maximum third party reimbursement through direct oversight of insurance verification and authorization processes.11.Gathers, collates, and reports referral statistics including key customer referral trends.12.Maintains comprehensive working knowledge of Meridian Home Health contractual relationships and ensures that patients are admitted according to contract provisions.13.Maintains comprehensive working knowledge of community resources and assists referral sources in accessing community resources should services not be provided by Meridian Home Health.The above statements are intended to be a representative summary of the major duties and responsibilities performed by incumbents of this job.
